Insurance:

If you have medical insurance and you wish for your insurance to pay for sessions, you will have to pay for the sessions first/upfront. I will provide you with receipts and statements of documentation for you to submit on your own behalf for reimbursement by your insurance. Super-bills for Insurance Claims are ONLY provided for full-pay of 60 ($155) and 90 Min ($180) sessions and NOT provided on packages, discounts, promos, home visits or Gift Cards/Certificates.

I WILL NOT SUBMIT CLAIMS ON YOUR BEHALF.

For 2022 - those receiving financial assistance, first responders, front line workers, online discounts and or promo’s for any service, packages, memberships, etc., are not eligible for insurance submission.

Coverage:

If you plan on utilizing your insurance to cover the costs of your sessions, please call your insurance FIRST and verify your coverage, and let me know in advance of your desire.

Check with your insurance company whether they cover Fascial Stretch Therapy or Massage Therapy. Insurance that covers massage therapy may cover stretching, since I am certified as a massage therapist in California - CA#86449.

It is rare that insurances will cover Fascial Stretch Therapy even though I am a certified massage therapist.

Prescription:

If you choose to utilize your insurance, be sure to also verify if your insurance will require an MD’s prescription.

PLEASE NOTE: I may require an MD’s prescription regardless of insurance approval or not, should I feel that you need clearance by an MD prior to being seen or at any point during our time working together should any medical issues arise. If you are in doubt - please call me so we can discuss your situation in detail to insure the best care possible.

I am more than happy to work with your medical team - MD, Chiro, Physical Therapist, Personal Trainer, etc., to insure quality continuum of care.

HSA’s / FSA’s:

Many clients use their HSA or FSA to pay for Fascial Stretch Therapy sessions. Please contact your provider and verify that Fascial Stretch Therapy or massage therapy are approved for spending from your account.

If you need a detailed receipt for documenting your HSA or FSA spending, please notify me in advance of your session to prepare paperwork.
